Support
Support plan
Your support plan tells staff what you can do, what you need support with and how you like to be supported. It is very important and is a legal document. We will work together to develop and follow your support plan.
Person-Centred Plan
We will support you to make a person-centred plan. This is a document that shows:
What is important to you
What you like
What you don’t like
Goals for your life
Your dreams
Your Home
Your opinions and thoughts are important. Once a month, we support people to have a tenants or residents meeting.
Everyone will sit in the lounge and discuss issues about your home and care. At the meeting everyone can choose topics to talk about.
Some topics to talk about are:
Health and safety
Problems at home
Problems with staff
Things you like
Things you are unhappy about
Complaints
The CQC
The Care Quality Commission (CQC) will stop poor care. On the CQC website you can see Care Opportunities’ inspection reports.
